Berberine and adiposity in diabetes-free individuals with obesity and MASLD (BRAVO)
337 diabetes-free adults with obesity and metabolic dysfunction-associated steatotic liver disease were randomised to 1 g per day of berberine or placebo for six months across 11 Chinese hospitals. Mean age 41.8 years, 65.6 percent male, adherence about 90 percent in both arms. Both primary endpoints were missed. Visceral fat area differed by 1.4 percent (97.5 percent CI −2.4 to 5.2) and liver fat content by 0.9 percent (97.5 percent CI −0.4 to 2.1). Lipids did move: LDL cholesterol −7.72 mg/dL (95 percent CI −13.13 to −1.93), apolipoprotein B −3.42 mg/dL (95 percent CI −6.33 to −0.51), high-sensitivity CRP −0.072 mg/dL (95 percent CI −0.140 to −0.004). Adverse events were similar between arms. The population was Chinese and mostly male, which limits how far it travels. Registered as NCT05647915.
Lipid-lowering effect of berberine in human subjects and rats
Obese adults took 500 mg of berberine three times daily for 12 weeks, with a parallel rat model. Triglycerides fell 23 percent and total cholesterol 12.2 percent, alongside mild weight loss averaging about 5 lb per person. In rats, triglycerides fell 34.7 percent and cholesterol 9 percent. CRP, IL-6, TNF-alpha, and thyroid markers did not change significantly, and liver, kidney, and cardiovascular function were unaffected. The human arm is small and the paper does not report its sample size, so read it as the origin of the lipid claim that BRAVO later confirmed on LDL cholesterol, not as strong evidence on its own.
Berberine and dihydroberberine inhibit mitochondrial respiratory complex I
The mechanism paper. Berberine and its more bioavailable derivative dihydroberberine inhibit complex I, which raises the AMP to ATP ratio and activates AMPK. That is the same target metformin hits. This is cell and rodent work with no doses or sample sizes reported in the abstract, not a human outcome trial, and it is the reason berberine gets called "nature's metformin" by people selling it.
How to choose.
- Split 900 mg to 1,500 mg across meals. Trials use 500 mg three times daily with food. Taking the full daily dose at once is the single most common cause of the cramping and diarrhoea people quit over. The BRAVO trial used 1 g per day.
- Standard berberine HCl absorbs at roughly 5 percent. Dihydroberberine and berberine phytosome formulations absorb better and are dosed lower as a result. If you buy one of those, do not also apply the 1,500 mg target. Match the dose to the form on the label.
- Third-party testing matters more here than usual. Berberine is extracted from plants such as Berberis and Coptis, which concentrate heavy metals. A dated batch-specific Certificate of Analysis from an independent lab covering heavy metals is the check to make. What a CoA is.
- Check interactions before you check price, and check Italy. Berberine interacts with metformin, statins, and blood thinners through CYP3A4 and P-glycoprotein. Trials use 900 mg to 1,500 mg per day of berberine HCl, split as 500 mg three times daily with meals. The 2026 BRAVO trial used 1 g per day for six months and did not reduce visceral or liver fat, though it did lower LDL cholesterol.
Does berberine reduce belly fat?
The largest trial says no. BRAVO randomised 337 adults with obesity and fatty liver to 1 g per day for six months and found no difference against placebo in visceral fat area (1.4 percent, 97.5 percent CI −2.4 to 5.2) or liver fat content. It did lower LDL cholesterol and apolipoprotein B.
Why does berberine cause stomach problems?
Standard berberine HCl absorbs at roughly 5 percent, so most of the dose stays in the gut. Splitting it into 500 mg doses with food is the fix. Dihydroberberine absorbs better and is usually easier tolerated.
Is berberine the same as metformin, and can I take both?
Not the same, and ask your prescriber before combining them. Both activate AMPK, and Turner N and colleagues showed in 2008 that berberine does it by inhibiting mitochondrial complex I, the same route as metformin. Berberine is not a licensed drug, its effect sizes are smaller, and it has no mortality outcome data. It also affects CYP3A4 and P-glycoprotein and can stack with metformin and statins, which is the strongest interaction warning of the four molecules on these pages.
Is berberine legal in Europe?
It is sold as a food supplement across most of the EU. Italy treats berberine as a medicine at 10 mg per dose and above, so Italian availability is restricted.
Is dihydroberberine worth the extra money?
It absorbs better than berberine HCl, so the effective dose is lower and gut side effects are usually milder. The human outcome trials, including BRAVO, were run on standard berberine, so the outcome evidence sits with the cheaper form.
