Mouse data is compelling. Human data is very limited, one small trial in COVID-19 patients, one in frailty. Longevity outcomes untested.

Best-supported claim
Fisetin's strongest human data is safety, not efficacy. Scott Tashman, Marc Philippon, Grant Dornan, and Johnny Huard's 2025 phase I/II randomized, dubbelblind, placebokontrollerad trial in 74 people with knee osteoarthritis found intermittent high-dose fisetin caused no significant increase in adverse events versus placebo across 12 months (https://www.ors.org/wp-content/uploads/AM25/429.pdf). That established the dosing strategy is tolerable. Beyond safety, fisetin's senolytic mechanism, clearing senescent cells, has been demonstrated in human tissue only for the related combination dasatinib plus quercetin, not fisetin alone, in LaTonya Hickson's 2019 diabetic kidney disease biopsy study (https://pmc.ncbi.nlm.nih.gov/articles/PMC6796530/).
Not supported by the evidence base
Marketers position fisetin as a proven senolytic that clears zombie cells and extends healthspan in humans, based on mouse lifespan data. No published human RCT has shown fisetin extends lifespan or clears senescent cells in human tissue on its own. The best-designed human efficacy trial to date, the 2025 knee osteoarthritis RCT, found no significant benefit on pain, cartilage MRI T2 relaxation, physical function, or gait after a year of treatment (https://www.ors.org/wp-content/uploads/AM25/429.pdf). A 2024 study by Eric Lee and colleagues found adding fisetin to dasatinib and quercetin did not clearly improve epigenetic aging markers over dasatinib and quercetin alone, and DQ alone actually increased epigenetic age acceleration on some clocks at 3 and 6 months (https://pubmed.ncbi.nlm.nih.gov/38393697/). Human efficacy trials in cancer survivor frailty (TROFFi) are still ongoing and unpublished as of 2026.

Fisetin was well tolerated in the 2025 knee osteoarthritis trial: 185 adverse events were reported across 74 subjects, mostly minor (joint pain, nausea, headache, dry mouth), with no significant difference between fisetin and placebo groups (all p>0.5). Four serious adverse events occurred, evenly split between groups, all from unrelated surgeries. Fisetin uses an intermittent hit-and-run dosing strategy rather than daily supplementation, following the senolytic model established in mouse studies. Long-term safety of chronic daily dosing has not been established.
