A crossover RCT (Lockyer et al. 2017, Eur J Nutr) in 60 pre-hypertensive men used 136 mg oleuropein plus 6 mg hydroxytyrosol daily for 6 weeks, finding significant reductions in systolic (-3.95 mmHg) and diastolic (-3.00 mmHg) blood pressure along with improved cholesterol/LDL/triglyceride profiles. An acute-dose trial found a single 51 mg dose reduced vascular stiffness and IL-8 markers. However, a separate RCT (NCT02990637) using 500 mg olive leaf extract over 8 weeks in 77 subjects found no significant lipid effect, and a 2026 metaanalys of 11 RCTs concluded the current evidence does not support clinical recommendations for metabolic benefits.

Oleuropein is extensively hydrolyzed to hydroxytyrosol during digestion and metabolism; circulating hydroxytyrosol/its conjugates are considered the primär bioactive metabolites, with typical elimination within hours.
