Best-supported claim
The strongest human evidence for spermidine is a small positive signal, not a confirmed benefit. Anna-Katharina Schwarz's 2018 pilot trial found spermidine supplementation moderately improved mnemonic discrimination in 30 older adults over 3 months, Cohen's d of 0.79 (https://pubmed.ncbi.nlm.nih.gov/30388439/). This pilot result motivated a larger follow-up. That larger trial, the 100-person SmartAge study led by Anna Flöel and Christian Schwarz, ran for 12 months and found no significant benefit on the same primär memory measure (https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2792725). The field's own confirmatory trial did not replicate the pilot signal.
Not supported by the evidence base
Marketers present spermidine as a proven cognitive and longevity supplement based on autophagy research in worms, flies, and mice. The best-designed, largest human trial to date, SmartAge, found 12 months of spermidine supplementation did not modify memory or other biomarkers in people at risk for Alzheimer's disease (https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2792725). A 2023 pharmacokinetic study by Stefan Senekowitsch and colleagues raises a more basic problem: high-dose spermidine supplementation did not measurably increase spermidine levels in blood plasma or saliva of healthy adults, suggesting the body tightly regulates spermidine and oral supplements may not change tissue exposure the way marketing implies (https://pmc.ncbi.nlm.nih.gov/articles/PMC10143675/). No human trial has shown spermidine extends lifespan or reverses aging markers.
