The NCT03464500 trial in older adults showed 12% more muscle strength after 4 months on 1000 mg/day, and that finding was later replicated. That kind of hard endpoint result is very rare in the longevity space.

Best-supported claim

Anurag Singh, Davide D'Amico, and colleagues at Amazentis ran the ATLAS trial, a randomized, placebokontrollerad study in 88 overweight, low-endurance middle-aged adults given 500 mg or 1000 mg of urolithin A (Mitopure) daily for 4 months (https://pmc.ncbi.nlm.nih.gov/articles/PMC9133463/). Leg muscle strength rose about 10 to 12% at both doses versus placebo, plasma acylcarnitines and C-reactive protein fell, and skeletal muscle biopsies showed increased mitophagy-related protein expression. A 2024 systematisk översikt by researchers pooling five studies and 250 participants confirmed urolithin A consistently increases muscle strength and endurance and lowers inflammation, but found no effect on maximal ATP production, mitochondrial biogenesis, body composition, or physical function tests (https://pubmed.ncbi.nlm.nih.gov/39002645/).

Not supported by the evidence base

Urolitin A is marketed as a comprehensive anti-aging compound for brain, skin, muscle, and cellular energy. Human RCT evidence supports only a narrow slice of this: modest muscle strength and endurance gains, plus some anti-inflammatory markers. The 2024 systematisk översikt explicitly found no effect on mitochondrial ATP production, biogenesis, or overall physical function tests, despite marketing language about 'boosting mitochondrial health' (https://pubmed.ncbi.nlm.nih.gov/39002645/). A 2026 review states plainly that no completed human trial has evaluated cognitive or neurodegenerative disease outcomes, so brain-health claims rest on mouse data alone (https://pubmed.ncbi.nlm.nih.gov/42468665/). The JAMA Network Open trial in older adults also missed its primär endpoints, hand-grip ATP production and 6-minute walk distance, with only secondary endurance measures reaching significance (https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2788244).

Urolitin A has been safe and well tolerated across every published trial to date, including 4-month dosing at 500-1000 mg/day in middle-aged and older adults. Preclinical toxicology found no genotoxic effects, with a no-observed-adverse-effect level of roughly 557-617 mg/kg bodyweight/day in human-equivalent terms. Long-term safety beyond several months has not been studied. Direct oral supplementation bypasses the variability of gut bacteria needed to convert dietary ellagitannins into urolithin A, so supplement-form doses reliably produce plasma levels about six times higher than pomegranate juice.
