This meta-analysis pooled 16 randomized controlled trials with 492 adults on creatine monohydrate and cognitive outcomes. Memory improved with a standardized mean difference of 0.31 (95% CI 0.18-0.44), attention time improved (SMD -0.31) and processing time improved (SMD -0.51), but overall cognitive function and executive function showed no significant benefit. Every included study used monohydrate. Certainty was moderate for memory and low for other domains. Effects were larger in older adults and at doses above 0.4 g/kg/day for 4+ weeks. Context caps enthusiasm: the entire evidence base is under 500 participants, the largest single preregistered trial (N=123 crossover, 5 g/day for 6 weeks) was mostly null with more biverkningar on creatine, and EFSA rejected a creatine-cognition health claim in 2025 citing insufficient evidence. An earlier memory meta-analysis (SMD 0.29) received a published double-counting critique. Reasonable read: small probable memory benefit, most plausible after age 60 or under metabolic stress; not a general brain booster.

Current evidence suggests that creatine monohydrate supplementation may confer beneficial effects on cognitive function in adults, particularly in the domains of memory, attention time, and information processing speed.
