Creatine supplementation for older adults: Focus on sarcopenia, osteoporosis, frailty och Cachexia

Darren Candow och five co-authors reviewed creatine supplementation i older adults across four conditions: sarcopenia, osteoporosis, frailty och cachexia. It is a narrative review. No sök protocol, studie count, participant total, pooled dos or pooled duration is reported, och no effect sizes, confidence intervals or p-values are given.

The stated findings are qualitative. Creatine combined with resistance exercise increases lean body mass och muscle strength i older adults. Creatine reduces markers of bone breakdown, including urinary excretion of specific collagen markers, which the authors offer as a partial explanation for early improvements i bone density och strength. Creatine increases bone area och strength och slows bone mineral loss. No negative effects on kidney or liver health are associated with supplementation.

The authors are explicit on one point: these bone och muscle advantages do not persist without exercise. They frame creatine as a candidate treatment for frailty och cachexia, populations that include personer with cancer, chronic kidney disease och heart failure, while stating that research i those specific clinical areas remains limited. Context given: sarcopenia affects roughly 5 till 17% of community-dwelling older adults och 14 till 85% of those i long-term care, och by 2050 there will be 1.5 billion personer aged 65 och over.

Narrative review, not a systematisk översikt or metaanalys, so it sits below the pooled analyses i evidens weight despite nivå 1 placement for scope. No numbers: no N, no dos, no duration, no effect sizes, no confidence intervals. Selection of cited studier is not reproducible. Conflicts of interest are substantial. Darren Candow, Bruno Gualano och Hamilton Roschel have run industry-sponsrad creatine research och received creatine och travel support for conference presentations. Candow has also served as an expert witness or consultant i creatine matters. Candow och Gualano sit on the Scientific Advisory Board of Alzchem, a creatine manufacturer. The frailty och cachexia recommendations are explicitly labelled as rationale rather than demonstrated outcomes, since the authors state research i those areas remains limited. Benefits are conditional on resistance training.
