Observational study in 78 participants (58 obstructive sleep apnea patients, 20 healthy controls). Plasma LL-37 measured 75.3 +/- 29.3 ng/L in the AHI < 15 group vs 47.1 +/- 18.3 ng/L in the AHI >= 15 group — roughly 37% lower with more severe apnea. LL-37 correlated negatively with age, %Stage N1 sleep, arousal index, AHI and oxygen desaturation index, and positively with %Stage N3, %REM, mean SpO2 and minimum SpO2. Multiple regression identified %Stage N1 as the independent predictor. No intervention, no dosing. This is endogenous-biomarker work: it says nothing about LL-37 supplementation.
