Liraglutide is an FDA-approved GLP-1 receptor agonist for type 2 diabetes (Victoza) and obesity (Saxenda). The LEADER trial showed it reduces cardiovascular death. It produces weight loss of 5-8% in obesity trials. It has extensive human safety data from over a decade of clinical use.

Liraglutide is the oldest GLP-1 receptor agonist in this comparison and the one with the longest real-world safety record. It is a prescription medicine approved for type 2 diabetes (Victoza) and chronic weight management (Saxenda).

The mechanism is shared with semaglutide. Liraglutide mimics the incretin hormone GLP-1, binding GLP-1 receptors on pancreatic beta cells to stimulate glucose-dependent insulin secretion. It suppresses glucagon, slows gastric emptying, and reduces appetite via central GLP-1 receptor activation. The key chemical difference from semaglutide is the shorter half-life (approximately 13 hours versus 165 hours), requiring once-daily rather than once-weekly dosing.

The SCALE Obesity and Prediabetes trial (N=3,731, 56 weeks) is the pivotal registration trial for the weight management indication. Patients were randomized 2:1 to liraglutide 3.0 mg daily (2,487 patients) versus placebo (1,244 patients), both with lifestyle counseling, in adults with BMI 30 or higher, or 27 or higher with dyslipidemia or hypertension. For comparative context, the PIONEER 4 trial comparing oral semaglutide to liraglutide reported liraglutide producing 3.1 kg weight loss over 52 weeks versus 4.4 kg with oral semaglutide and 0.5 kg with placebo. Liraglutide's weight-loss magnitude is consistently the smallest of the four molecules compared in head-to-head and cross-trial contexts.

No dedicated cardiovascular outcomes trial equivalent to semaglutide's SELECT was identified for liraglutide as a weight-management agent in this research. Liraglutide's diabetes-indication cardiovascular outcomes trial (LEADER) was not directly fetched and verified in this research pass.

Body composition data is a gap. No liraglutide-specific DXA or lean-mass to fat-mass body composition trial data were found and verified via PubMed or PMC, unlike the substantial substudy data available for tirzepatide and retatrutide. This is a significant limitation for a longevity-focused comparison where body composition matters.

Safety is the most established of the four molecules given 16 years of real-world use since 2010. Gastrointestinal adverse effects (nausea, vomiting, diarrhea) are common. Saxenda's FDA label includes a required discontinuation rule: treatment should be stopped after 12 weeks on the 3.0 mg daily dose if patients have not lost at least 5% of initial body weight. As with other GLP-1 receptor agonists, liraglutide carries labeling precautions regarding thyroid C-cell tumors seen in rodent studies.

Dosing is once-daily subcutaneous injection. Saxenda dose escalation: Week 1 at 0.6 mg daily, increasing weekly through 1.2, 1.8, 2.4 mg to a maintenance dose of 3.0 mg daily at Week 5. Victoza (diabetes) uses a lower dose range, typically up to 1.8 mg daily.

For longevity, liraglutide's relevance is primarily its long safety record and shared GLP-1 class mechanisms. Class-wide anti-inflammatory signaling (NF-kB and NLRP3 modulation) and NASH resolution benefits (OR 4.45 across the GLP-1 class) are pharmacologically plausible for liraglutide, though the supporting systematic review pooled data across the class rather than isolating liraglutide-specific effect sizes. The shorter duration of action (daily dosing) and comparatively smaller weight-loss magnitude make it the least potent option of the four for longevity applications centered on significant weight and metabolic risk-factor reduction.
