Stem Cells for Hair Loss
A 2024 systematic review of 12 RCTs and 514 patients found stem cell and stem-cell-secretome injections modestly boost hair density, but no randomized trial of exosomes for pattern hair loss existed as of that review, and clinics still sell exosome hair packages off that evidence gap.
Androgenetic alopecia (male and female pattern hair loss) is a common target for stem cell marketing because minoxidil and finasteride plateau for many patients and hair transplant surgery is expensive. The evidence base is real but small, fragmented across cell sources, and dominated by short follow-up windows.
The most complete synthesis is a February 2024 systematic review in Plastic and Reconstructive Surgery Global Open by Konstantinos Gasteratos, Konstantinos Kouzounis, and Jeremy Goverman, covering 12 randomized controlled trials and 514 patients with androgenetic alopecia published between 2013 and 2023 [1]. The trials used five different cellular approaches (hair-follicle-derived cells, adipose-derived stromal vascular fraction, bone-marrow mononuclear cells, umbilical-cord-blood MSC paracrine factors, and deciduous-tooth stem cells) and eight acellular, secretome-based approaches; the authors did not pool a single overall percentage improvement because the studies were too heterogeneous to combine [1]. Individual results ranged from clearly positive to marginal. Gentile et al. (2020, 27 patients) found hair-follicle-derived micrografts added 18.0 hairs per 0.65 cm² at 58 weeks versus a 1.1-hair decrease in the saline control area, p<0.0001 [1]. Kim et al. (2021, nine patients) reported a 48.11% hair density increase at the injected site with autologous adipose-derived stromal vascular fraction versus 35.48% at an untreated site [1]. Elmaadawi et al. (2018, 20 patients resistant to conventional therapy) found bone-marrow mononuclear cells produced 52% mean improvement versus 42% for follicular stem cells, a difference that did not reach significance, p=0.426, and came with a cost: 80% of subjects developed bone pain and hematomas from the marrow aspiration itself [1]. The review's own conclusion: stem cell-derived treatments were "safe and effective in improving hair regeneration and density," outcomes were sometimes temporary, and they may become useful add-ons to hair transplant surgery rather than a replacement for it [1].
Exosomes are the newer and more heavily marketed offshoot, and the evidence lags well behind the sales pitch. The 2024 Gasteratos review states directly: "currently, there are no published RCTs on exosomes for AGA" [1]. That gap has started to close since. A September 2025 systematic review in Clinical, Cosmetic and Investigational Dermatology by Mohammed Al Ameer and colleagues screened 2,178 records and found 11 clinical studies on exosomes for hair regrowth across all alopecia types, only two of them randomized controlled trials, covering 298 total participants with sample sizes as small as one patient in the weakest included study [2]. Hair density improvements across the included studies ranged from 9.5 to 35 hairs per cm², with hair thickness gains up to 13.01 micrometers, and no serious adverse events were reported in that specific dataset [2]. One of the two RCTs, Amini et al. (2025, Malaysia, 20 men, Norwood grade 2-3), found a plant-extract-combined exosome product produced a median 9.5-hair increase versus 1.5 hairs with placebo at 12 weeks, p=0.006, but 20% of treated patients developed mild scalp irritation [2].
That thin trial base has not slowed commercial rollout. Exosome-containing scalp injections and "exosome hair boosters" are widely advertised at medical spas and hair clinics, frequently alongside PRP and microneedling, and frequently marketed with the implicit claim that they work like stem cells without the regulatory baggage of a stem cell product. Documented harm from unregulated cosmetic exosome injections includes persistent erythema, granulomatous nodules, and scarring in a 2025 case series of four women treated at non-dermatology clinics or beauty salons [4].
Practical takeaway: stem cell and stem-cell-secretome injections for pattern hair loss have real, published, randomized evidence behind them, mostly from small trials (20 to 84 patients) with 6-to-58-week follow-up, and modest but statistically real hair-count gains over placebo in several of them [1]. Exosome-specific injections for hair loss have far less RCT evidence, none at all as recently as the 2024 review, and the product a patient receives at most clinics is not standardized, not FDA-approved, and not always disclosed by source or dose [1][2][3]. Anyone considering either option should ask what cell type or exosome source was used, request the specific trial or product name behind the clinic's claimed results, and treat this as an unapproved, investigational category rather than a proven alternative to minoxidil, finasteride, or transplant surgery.
