GRADE-assessed meta-analysis of 7 randomised controlled trials with 8,069 participants comparing CagriSema (cagrilintide + semaglutide co-formulation) against active comparators and placebo. Weight loss vs semaglutide: mean difference -7.58 kg (95% CI -10.30 to -4.86, p<0.00001). Vs cagrilintide: -9.24 kg (95% CI -10.46 to -8.02, p<0.00001). Vs placebo: -13.99 kg (95% CI -18.38 to -9.61, p<0.00001). Glycaemic outcomes were heterogeneous across trials. Lipid parameters improved vs placebo. Adverse events were mainly gastrointestinal and injection-site reactions; serious adverse events did not increase vs comparators. CagriSema remains unapproved; regulatory decision expected Q4 2026.

CagriSema produced significantly greater weight loss than semaglutide, cagrilintide, and placebo across 7 RCTs.
Critic notes

Meta-analysis of trials in mixed populations; most component trials are sponsor-funded. Head-to-head comparisons with semaglutide rely on a limited number of trials.
