Resveratrol for the management of human health: how far have we come? A systematic review of resveratrol clinical trials
137 published RCT reports, 104 unique randomised trials, more than 4,800 people given resveratrol. 92 of the 137 reports (67 percent) found a significant benefit on their primary outcome and 42 (31 percent) found none. Daily doses spanned 5 mg to 5,000 mg, a thousandfold range, with 500 mg the most studied at 32 trials. Only about 5 percent of trials gave resveratrol to more than 100 people. The authors state their own conclusion plainly: there is no conclusive clinical evidence to recommend resveratrol in any healthcare setting.
Impact of resveratrol supplementation on human sirtuin 1: a GRADE-assessed systematic review and dose-response meta-analysis of randomized controlled trials
Eleven RCTs measured SIRT1 directly: four for gene expression, five for protein expression, three for serum levels. Gene expression, standardised mean difference 0.05, 95 percent CI −0.24 to 0.34, P = .73. Protein expression, 0.30, 95 percent CI −0.15 to 0.77, P = .18. Serum SIRT1, mean difference −0.04, 95 percent CI −0.235 to 0.16, P = .70. All three null. A subgroup of trials shorter than 12 weeks did show a gene-expression increase. This is the paper to read if you are buying resveratrol because of sirtuins.
Health effects of resveratrol: results from human intervention trials
The 52-week phase II trial in Alzheimer's disease (n=119) escalated from 500 mg per day to 1,000 mg twice daily. Resveratrol was safe and well tolerated and it lowered MMP9 in cerebrospinal fluid against placebo. Aβ40 fell in both arms, so that drop was not the drug. Brain volume decreased faster in the resveratrol group for reasons the authors could not explain. A PREDIMED analysis of 1,000 high-cardiovascular-risk adults found higher urinary resveratrol metabolites tracked with lower fasting glucose, triglycerides, and heart rate, with no association for cholesterol or blood pressure. That arm was observational, not an intervention.
How to choose.
- Buy on labelled trans-resveratrol, not "resveratrol". Cis-resveratrol is biologically inactive. A label that says only "resveratrol 500 mg" tells you nothing about how much of it is the active isomer. Brands that test their material state trans-resveratrol content as a percentage, usually 98 percent or higher. Anything vaguer is unmeasurable.
- Target 150 mg to 1,000 mg per day. That is the range published human trials used. The 2024 systematic review found 500 mg was the single most studied dose, used in 32 trials, and that 88 percent of trial arms took 1 g per day or less. Higher is not better: gastrointestinal upset climbs above 1,000 mg per day and bioavailability does not.
- Insist on a dated third-party Certificate of Analysis. Independent labs only. Eurofins, Alkemist, NSF. A manufacturer testing its own powder is not third-party testing. The CoA should be batch-specific and dated within 12 months. What a CoA is.
- EU status is clear, which makes shipping simple. Trans-resveratrol is an authorised novel food in the EU under Regulation 2016/1866 for use in food supplements. This is a much easier regulatory position than NMN, which has no EU authorisation. See the current regulatory tracker.

How much resveratrol should I take per day?
Published human trials used 150 mg to 1,000 mg per day, with 500 mg the most common dose. Doses above 1,000 mg per day raise gastrointestinal side effects without better plasma levels.
Does resveratrol actually activate SIRT1 in humans?
The pooled evidence says no. Mansouri F and colleagues pooled 11 RCTs in 2025 and found no significant effect on SIRT1 gene expression (P = .73), protein expression (P = .18), or serum levels (P = .70).
Is trans-resveratrol better than regular resveratrol?
Trans-resveratrol is the active isomer. Cis-resveratrol is inactive. Any product worth buying states trans-resveratrol content on the label, typically 98 percent or higher.
Is resveratrol legal to buy in the EU?
Yes. Trans-resveratrol is an authorised novel food in the EU under Regulation 2016/1866 for food supplement use, and it is authorised in the UK.
Why is resveratrol only evidence grade C?
The trial count is high but the results conflict, effect sizes are small, and oral bioavailability is poor. Grade C means mixed or small human trials. Berberine and urolithin A both score higher on this site with far fewer studies.
Should I buy resveratrol with piperine?
Piperine slows resveratrol glucuronidation in animal work and some brands add it for that reason. There is no human RCT showing the combination produces a better clinical outcome than resveratrol alone.
