Urolithin A improves muscle strength, exercise performance, and biomarkers of mitochondrial health

Registered as NCT03464500. 90 middle-aged, overweight, inactive adults were randomised to 500 mg of urolithin A, 1,000 mg, or placebo for four months. Muscle strength improved roughly 12 percent. Peak VO2 and six-minute walk distance improved. Plasma acylcarnitines and C-reactive protein fell. The registered primary endpoint, change in cycle-ergometer power output at day 120, was not significantly improved, so the strength and endurance findings that drive the marketing were secondary. Funded and co-authored by Amazentis, which sells Mitopure.

ATLAS 2: a confirmatory trial of urolithin A on muscle strength and performance

A registration, not a result. 120 healthy adults aged 40 to 65 with BMI 25 to 34.9 take placebo, 500 mg, or 1,000 mg of Mitopure daily for six months. The primary endpoint is change in maximum isokinetic knee flexion strength measured by Biodex dynamometry, a direct strength measure rather than the power output the 2022 trial missed on. Sponsored by Amazentis, which sells the ingredient. Estimated completion October 2027. Until it reports, this category rests on one four-month trial.

How to choose.

  1. Buy direct urolithin A, never a "precursor". Pomegranate extract only becomes urolithin A if you carry the right gut bacteria, and roughly 40 percent of people do. A pomegranate or ellagitannin product is a bet on your microbiome. Direct urolithin A skips the step, which is the entire point of the supplement.
  2. Target 500 mg to 1,000 mg per day. The 2022 trial dosed at 500 mg and 1,000 mg for four months and ATLAS 2 is testing the same two doses. Both sit inside the range with human data behind them. No published trial shows 1,000 mg beating 500 mg, so paying double for the higher dose buys no evidence.
  3. Check the CoA for urolithin A content, not extract weight. This is a synthesised molecule with a small number of suppliers. A dated batch-specific Certificate of Analysis from an independent lab is the only way to confirm what is in the capsule. What a CoA is.

Is pomegranate extract the same as urolithin A?

No. Pomegranate ellagitannins convert to urolithin A only in people with the right gut bacteria, roughly 40 percent of the population. Direct urolithin A works regardless of your microbiome.

Did the urolithin A trial actually work?

Partly. Muscle strength rose about 12 percent and endurance measures improved in 90 adults over four months, but the registered primary endpoint, cycle-ergometer power output at day 120, was not significantly improved. The strength result was a secondary endpoint.

Is Mitopure the only real urolithin A?

Mitopure is Amazentis's branded urolithin A and it is the material used in the published trials. Other brands sell urolithin A that is not Mitopure. Judge those on the Certificate of Analysis, which is why third-party testing is weighted in the score.

Is urolithin A approved in the EU?
