En systematisk översikt och metaanalys från 2026 analyserade 13 RCTs (n=757 deltagare) av väterikt vatten hos vuxna med BMI >=25. HRW minskade totalkolesterol signifikant (WMD: -6.71 mg/dL, 95% CI: -10.38 till -3.04, p<0.001) och LDL-C (WMD: -3.21 mg/dL, 95% CI: -6.31 till -0.10, p=0.043). Effekterna på triglycerider och HDL-C var inte signifikanta. The authors explicitly stated that the magnitude of these changes is below clinically meaningful thresholds for cardiovascular risk reduction, and that current evidence does not support routine use of HRW as a lipid-lowering therapy. The review searched Scopus, PubMed, Web of Science, and Embase for RCTs published up to november 2025.

HRW supplementation in overweight or obese adults produces small, statistically detectable but clinically modest reductions in TC and LDL-C, with minimal effects on TG and HDL-C. These effects are below thresholds considered meaningful for cardiovascular risk reduction and should not justify routine use of HRW as a lipid-lowering therapy outside of research settings.

The effect sizes, while statistically significant, are clinically negligible — a 6.71 mg/dL TC reduction is less than what a single serving of oatmeal achieves. The authors correctly noted this. Heterogeneity across studies was not fully explored. Most included RCTs were small (N<50).
